- Patient visit
- Patient receives brochure
- Discuss billing
- Document consent in the Electronic Medical Record (EMR)
Day 1-3:
Clinical Team
- Send a referral to YHS
- Schedule an appointment
Day 3:
YHS
- Face-to-face appointment
- Tele-appointments may be arranged
Day 4: Care
Management Team
- YHS Consultant goes over the assessment with a case manager
Day 5:
Follow-Up
- The patient will be called weekly during the first month, then as needed
Transition of Care (TOC)
Coordination of continuity of healthcare that helps reduce hospital readmissions and improves communication between patients and their providers.
Day 1:
Nurse
- Moderate to high-risk patient added to TOC nurse queue
Day 1-2:
Nurse
- TOC nurse identifies high-medication-risk patients
- TOC nurse sends a referral to YHS consultant
Day 2-3:
YHS
- YHS contacts the patient within 48 hours
Day 7-14:
YHS
- YHS schedules a face-to-face appointment with the patient within 7 to 14 days
Medication Therapy Management (MTM)
Optimization of therapeutic outcomes while reducing patient costs and improving quality of life.
Day 1:
Nurse
- The nurse runs a monthly report on patient adherence
Day 1-2:

- Patients are referred to YHS consultants

- YHS consultant sets up an appointment with the patient
